Returning D124 Families

Welcome back Evergreen Park Elementary School District 124 Parents

Enrollment is open to all legal residents who reside within the school boundaries of Evergreen Park. All District 124 families are required to prove residency and pay school fees annually. This year, district families may verify their residency and pay fees online as part of the registration process if both of the following two conditions:

  • The student(s) being registered was/were students in a D124 school during the 2017-18 school year and/or your student(s) were pre-registered during Kindergarten registration, AND
  • The parent/guardian enrolling the student(s) for the 2018-19 school year is a homeowner.

Parents and guardians who are renting a home or apartment, and those who are living with a District 124 resident may complete the registration process online, but will still need to make an appointment to verify residency within District 124 boundaries. In addition, all families who have changed their address since the beginning of the preceding school year must visit the District Administrative Office to verify residency at their new address prior to August 9, 2018.

ACCEPTABLE RESIDENCY VERIFICATION ITEMS: INCLUDE ONE ITEM OF EACH OF THE FOLLOWING GROUPS: 

For Homeowners:

  • Current mortgage statement or homeowner’s insurance policy.
  • One current utility, telephone, or water bill, current bank or credit card statement, or current paycheck stub (dated July 1, 2018 or later) that contains the registrant’s name and address. 

For Renters:

  • Certificate of Inspection for the property from the Village of Evergreen Park.
  • Signed Apartment lease listing all tenants
  • Registrant’s receipt from the previous month’s rent payment (July 2018)
  • One current utility, telephone, or water bill, current bank or credit card statement, or current paycheck stub (dated July 1, 2018 or later) that contains the registrant’s name and address.

Re-Registering Current Student(s) for the Next School Year 

Parents should have received a letter and/or email with a SnapCode for each District 124 student that contains a personalized link to complete the online registration and fee payment for the 2018-2019 school year. We appreciate all families to take action and complete the online registration as soon as possible once the enrollment period opens. After you have completed the registration process online, you must complete the verification of residency. A student’s registration will not be considered complete until all the forms and proof of residency have been submitted.

Registering a New Student for the Next School Year (Pre-K, Kindergarten)

Parents that pre-registered their student(s) for Kindergarten and/or are eligible for the Pre-Kindergarten program should have received a letter and/or email with a SnapCode for each child that contains a personalized link to complete the online registration and fee payment for the 2018-2019 school year.  After you have completed the registration process online, you must complete the verification of residency.  A student’s registration will not be considered complete until all the forms and proof of residency have been submitted.

ADDITIONAL FORMS (If applicable)

Letter of Residence Affidavit – All families who are living with a District 124 resident, for purposes other than school attendance, may complete the registration process online.  However, the district resident and parent/guardian must complete and sign these forms as a component of proof of residency and will need to make an appointment to verify residency within District 124 boundaries.

Change of Residence – If a family moves within the boundaries of a different District 124 school, the student may complete the year at the original school of attendance.  Parents are required to inform and provide the new address to the school of attendance and complete a change of address form.  All families who have changed their address since the beginning of the preceding school year must visit the District Administrative Office to verify residency at their new address.  Please print forms if applicable to you. 

In-District Transfer Request – Parents/Guardians who wish their child to attend a District 124 school outside of their home boundary may submit a written request by August 2, 2018 (for the following school year 2018-2019).  Requests for transfers are made each year and parents are responsible for transportation.  The parent or guardian requesting the transfer will be notified prior to the start of the school year if the transfer has been approved or not.  Request will be taken into consideration after the general registration process have been completed.  Request must be taken to the District Administrative Center addressed to District 124 Superintendent.
