D124 uses an online registration process through the PowerSchool system for all returning and new students.

All District 124 families are required to prove residency and pay school fees annually.


Register in March 2025 to receive a 50% discount on fees!

Enrollment is open to all residents who reside within the school boundaries of D124. Register Online. You will receive emails from the district registrar every step of the way.

Returning Student Information 

What happens after I register?

After you have enrolled your student(s) online, District 124 will continue to use the CLEAR Batch system to electronically review student residency. The CLEAR Batch system reviews public records to verify residency. ALL returning students will be reviewed using this system and will receive notification of your residency status via email and a postcard.

If additional documentation for proof of residency is needed, households will be contacted via email by April 14. Parents MUST attend Open Registration or make a residency appointment to prove residency

D124 will be hosting an open registration event for those who need to prove residency on Tuesday, April 29 at CMS for 6th, 7th and families with children at multiple schools and Wednesday, April 30 at each Elementary School for K through 5th) from 4-7 p.m. Only those who need to verify residence should attend these events.

 

What if my address has changed?

If your family has moved within the boundaries of a different D124 school, since the beginning of the preceding school year, parents are required to inform and provide the new address to the registrar before starting enrollment for your student(s). A Certificate of Residence (Appendix A) must be completed along with original proof of residency documents supplied.

 

What if we are living with another Evergreen Park District resident?

 If the parent/guardian and family are residing with another family within the district, the person who owns or rents the residence must complete and notarize a Student Residency Affidavit (Appendix C) form.  The property owner/lessee is not required to be present, however, COPIES of the following: one (1) document under category A, (2) documents of category B AND copy of district resident’s Driver’s License/State ID must be presented along with the affidavit.  The parent/guardian must provide three (3) documents of category B.

 

What if we do not plan to return to D124?

If your child/children WILL NOT be returning to District 124 for the 2025-26 school year, please call the school secretary to begin the proper student transfer out paperwork.

New Student Information 

What happens after I register?

After you have registered your student(s), an appointment to meet with the registrar will be required. Email [email protected] or call at (708) 423-0950 ext. 2145 to schedule an in-person appointment.

New student(s) Checklist (Required documents to register)

  • Original Birth Certificate with county seal.
  • Current Illinois Driver’s License/State ID, Passport or Consular ID Card (Out of Country)
  • Student Transfer Form ISBE Form 33-78 from previous school (if child is transferring from another public school in Illinois). The State of Illinois requires all students, including out of state, private school or parochial schools, provide certification in writing, on school or district letterhead, that the student is not currently serving a suspension or expulsion imposed by the school and what grade the student will be entering for the current school year.
  • Please obtain copies of your student health file from your previous school. The State of Illinois requires all students, including out of state or private school, to have a complete and current Illinois Certificate of Health, Proof of School Dental and an Eye Examination. Forms are due the first day of school or within 20 days of enrolling to our district, otherwise, we are required by the State to exclude your child until paperwork is received.
    • Certificate of Health and Immunization Record (Physical Form): Preschool, Kindergarten and 6th Grade
    • Dental Examination (Dental Form): Kindergarten, 2nd Grade and 6th Grade
    • Vision/Eye Examination (Vision Form): Kindergarten Only

  • Certificate of Residency (Appendix A) Form.
  • Release of Records form for student(s) who previously attended an elementary school in Illinois or Out of State.
  • Proof of Residency: Please review the Residency Page for residency registration requirements. You must provide documentation showing you live at an address within district boundaries.
  • Copies of any IFSP, IEP or 504 documents from previous district or any records necessary for a special placement (if applicable)
  • Present custody court order, agreement, judgment, or decree that awards or gives custody of the child/children (if applicable)

How do I pay fees?

For the 2025-26 school year, the registration fees will remain the same as prior year’s fees.  Families who complete the online returning student registration from March 3rd – March 30, 2025 will receive 50% off their registration fees. Parents/guardians will be able to pay fees through the online registration process or can pay via check or RevTrack during open registration days.
