Thank you for choosing District 124. Any parents/guardians who wishes to enroll their student(s) in Evergreen Park Elementary School District 124 must complete the online registration process for each student. Enrollment is open to all legal residents who reside within the school boundaries of Evergreen Park.
There are three steps required to complete registration before a student can be placed into a classroom:
Registering a New Student for the Next School Year
We encourage families to complete the Online Registration and schedule an appointment as soon as possible. Your student’s registration will not be complete until you have submitted the online registration, proved residency, and paid school fees. You will receive a confirmation email after the submission.
After you have registered your student(s), you must complete the Student Residency and Certificate of Residency affidavits, and schedule an appointment to prove residency. Click on Residency to get a list of acceptable proof of residency documents. Acceptable residency verification items include one document from Category A and two documents from Category B. Documents must be dated within the current month or within the last 30 days.
District 124 offers fee waivers for families who meet the eligibility criteria. Free/Reduced Lunch applications and Waiver of Fee forms (Spanish) will be available for parents during the residency appointment. Click here to view a list of current fees.

Student Residency Affidavit - A parent/guardian who wishes to enroll their student(s) in Evergreen park Elementary School District 124 must fully complete this form. Enrollment is open to all legal residents who reside within the school boundaries of District 124.
Certificate of Residency - Must complete and sign an affidavit as a component of proof of residency. This form must be submitted along with residency documents during registration before you student receives a class assignment.
Letter of Residence Affidavit - All families who are living with a District 124 resident, for purposes other than school attendance, may complete the registration process online. However, the district resident and parent/guardian must complete and sign these forms as a component of proof of residency and will need to make an appointment to verify residency within District 124 boundaries.
Evidence of Non-Parent Custody/Control - Must complete and sign form if a person other than the student’s parent or legal guardian has assumed custody, for purposes other than school attendance, and wishes to enroll the student.
Release of Records Form - If your student(s) previously attended an elementary school, this form must be printed, completed and provided during registration.
Health Forms - The State of Illinois requires all students to have a complete and current Certificate of Health, Proof of School Dental and an Eye Examination. If your student(s) will be starting Preschool, Early Childhood, Kindergarten, 2nd grade and 6th grade the health forms below are required. Forms are due the first day of school or within 20 days of enrolling to our district, otherwise, we are required by the State to exclude your child until paperwork is received.
