Employees who miss work for specified reasons related to COVID-19 are eligible for additional paid sick leave or expanded family and medical leave, through the Families First Coronavirus Response Act (FFCRA). The act is currently valid through the end of the calendar year on December 31, 2020.
Employees requesting a COVID-19-Related Leave should complete the COVID-19-Related Leave Request Form and return it to Business Office Clerk Peg Hammer at firstname.lastname@example.org, or to the District Office at 2929 W. 87th Street.
Employees planning to take a COVID-19 Related Leave should continue to record absences as sick days using the Aesop Absence Management System. Your sick days will be reimbursed following the approval of the COVID-19 Leave Request.
The form is also accessible through iVisions, and under the “Staff” dropdown menu on the District 124 website by clicking the “COVID-19 Leave Request Form.”
Families First Coronavirus Response Act
If caring for yourself:
Employees are eligible for up to two weeks (80 hours) of paid sick leave at the employee’s regular rate of pay if unable to work (or telework) and experiencing COVID-19 symptoms and seeking a medical diagnosis, and/or quarantined pursuant to Federal, State, or local government order or advice of a health care provider. The option to work from home must be pre-approved by your supervisor.
If caring for someone else:
Employees are eligible for up to two weeks (80 hours) of paid sick leave at two-thirds of the employee’s regular rate of pay (max $200/day) if the employee is unable to work or telework because of a bona fide need to care for an individual subject to quarantine (pursuant to Federal, State, or local government order or advice of a health care provider), or to care for a child (under 18 years of age) whose school or child care provider is closed or unavailable for reasons related to COVID-19; and
