Employees who miss work for specified reasons related to COVID-19 are eligible for additional paid sick leave or expanded family and medical leave, through the Families First Coronavirus Response Act (FFCRA). The act is currently valid through the end of the calendar year on December 31, 2020.
Employees requesting a COVID-19-Related Leave should complete the COVID-19-Related Leave Request Form and return it to Business Office Clerk Peg Hammer at email@example.com, or to the District Office at 2929 W. 87th Street.
Employees planning to take a COVID-19 Related Leave should continue to record absences as sick days using the Aesop Absence Management System. Your sick days will be reimbursed following the approval of the COVID-19 Leave Request.
The form is also accessible through iVisions, and under the “Staff” dropdown menu on the District 124 website by clicking the “COVID-19 Leave Request Form.”
