STUDENT REGISTRATION INFORMATION

 

 

Kindergarten and New Students

 

Kindergarten students must be 5 years of age on or before September 1, 2009.  Student Registration, for Kindergarten and students new to District 124, will take place on August 12, 2009 from 1:00-3:00 p.m. or 5:00-7:00 p.m.

 

Please bring your child’s original birth certificate (with county seal), social security number and documents listed below.

 

 

Returning Students

 

This year we are continuing our residency verification process and fee payment days. Registration packets were sent home, with students, to parents/guardians during the week of June 1st.  The packets contained a fee statement for each child, emergency information form/residency verification information.  The fee statement and emergency form/residency verification information, along with two proofs of residency from the list below, must be presented at registration. 

 

 

RESIDENCY VERIFICATION INFORMATION:

 

Required Documents for Kindergarten and New Students:

 

You will need one of the following: a copy of your mortgage papers, current homeowner’s insurance policy, apartment lease listing all tenants/certificate of inspection.  Additionally, residency photo ID card, and two (2) additional proofs of residency from list below must be presented at registration.

 

Acceptable Residency Verification for All Students:

 

One (1) utility, credit card or telephone bill and one (1) bank statement, mortgage statement, pay check stub (with address), or medicard statement  dated the month of July.  In addition to the above verification, all renters must also bring in a current lease.

Important school information (class assignments/schedules etc.) may be picked up in all buildings on Thursday, August 20, 2009 from 11:30 a.m. – 3:30 p.m. and 4:30 p.m. – 6:00 p.m.  Your child’s information will not be made available to you without residency confirmation.  Parents must attend one of the dates listed above prior to August 20, 2009 to avoid delays on that day.

EYE AND DENTAL EXAMS

 

All children enrolling in kindergarten, and any child enrolling in school for the first time, must provide proof of an eye examination before October 15, 2009.

 

Students in kindergarten, second, and sixth grade, and any child enrolling in school for the first time, must have a dental exam prior to May 15, 2010.

School District 124 offers Before and After School programs at each elementary school.  The fees for the Program are as follows:

There are two billing rates for the Extended Day Program – Scheduled and Drop-In.  Scheduled fees (listed above) are billed monthly and due in advance.  Drop-In fees (slightly higher) are billed the week following the student’s attendance.
